The arrival of the Lord of Hatred expansion finally brings a long-awaited legend to Diablo IV.
The expansion was released on April 28, 2026, and is the second major expansion for Diablo IV. The storyline revolves around Mephisto, the Prime Evil of Hatred, and serves as a major chapter of his arc.
Diablo II's infamous Cow Level has finally been added to Diablo IV as part of the Lord of Hatred expansion. The Secret Cow Level might only be accessible on a Tuesday, as per YouTuber and streamer "LoatheBurger."
To gain access to the Cow Level, you must collect three items, found across the base game, the Vessel of Hatred expansion, and the Lord of Hatred expansion.
How to Collect the Three Items?
The Stamina Potion (Diablo IV): In the original version, find and slay 666 cows across Kehjistan, Dry Steppes, and Scosglen to collect the Bloody Wooden Shard, Intricate Metallic Fragment, and Musty Tome.
Gain the Strange Key after cleansing the three relics in the Ked Bardu fountain, and use it to enter the Forlorn Hovel in the Scosglen region. Slaying the cows inside will drop the Stamina Potion.
Rusted Bardiche (Vessel of Hatred): While this also involves more cow-related tasks, players need to acquire three scrolls and hunt various statues in the Nahantu region.
Visit the gravesite of Onucha Ikha, located in the southwestern part of Kurack Docks. Use the "Thank You" emote in front of the central, northern, and southern statues to get the Jabbering Gemstone.
For the Crooked Staff, players must find Bloodbolt, an elite Lacuni enemy usually located in the eastern part of The Den in the Great Shelf.
Defeat it using the basic skill acquired at Level 1 and nothing else to get the Crooked Staff. To complete this, players must reset all skill points and dismiss any mercenaries before taking on Bloodbolt.
The Five Hills subzone has 12 bird statues that players must interact with in a specific order before returning to the first statue. Completing this successfully will drop the Rusted Old Bell.
Eliminating three Cordycepic Zombies will drop mushrooms. Drop the three relics into them and do not pick them up until they start emitting purple smoke. Offer these relics to the braziers at the entrance of the Oka'bo Temple to obtain the Unusual Key.
Use the Unusual Key to enter the Forlorn Burrow, where players must find a hidden breakable wall. Break the wall, enter, and eliminate the cows inside to get the Rusted Bardiche.
Neyrelle's Hand (Lord of Hatred): Read Outcast's Journal at Mount Hefeatrus to start a hidden quest that yields Neyrelle's Hand.
Once you have read the journal, find the Holy Cedar Tablets and inscribe "Fear," "Anger," and "Pride" in a specific order. Players must also complete specific trials.
Players are then required to follow a Large Crow in the Ruins of Broken Reason to get the Hierophant’s Skull, then fish in Hawezar to find the Locked Wooden Coffin containing Neyrelle's Hand.
Find Cow Island and Enter the Secret Cow Level
Put the three items into the Horadric Cube to obtain a Torn Page. Return to where the Holy Cedar Tablets were and inscribe "Greed" on them. Then, players must use the Outcast's Boat to reach Cow Island from Zerae's Shore.
Upon reaching Cow Island, players must enter the Hecatombs and eliminate all the cows to open the portal that will take them to the Secret Cow Level.
Defeat the Cow King to obtain the Cow King's Crown, a mythical helmet that grants day-specific buffs. While these day-specific buffs add a touch of humor, they offer little tactical advantage for high-level play.
